Use this form to notify the club of a forfeit as soon as possible.

Deadline — Friday 4pm

Forfeit notices must be submitted via this form by 4pm Friday for the upcoming round.

For forfeits after Friday 4pm, this form is too late — the manager must contact the club secretary (seaforthsecretary@gmail.com) directly so the day’s draw can be adjusted in time.
