Seaforth Netball is covered under Netball Australia's insurance provider, Howden.

If medical treatment is required or you wish to claim expenses, you must submit a claim directly through Howden.

The club injury form above and an official Howden / Netball Australia insurance claim are separate — you can submit the club report without making a claim, and a claim is not required to complete this form.
